Within the last two months, have you had any jaundice, anaemia, pneumonia, bronchitis, respiratory ailment or any surgery or any history of medical injury or sickness__ YES/NO (tick whichever is applicable)




RULES & REGULATIONS

  • The Karate uniform should always be kept clean and tidy
  • Finger and toe nails should always be well cut and trimmed
  • Please carry your OWN water bottle, towel and mat for each class and ensure to take care of your belongings
  • Karate GI (Dress) should be purchased at the time of admission

Admission & Fees
  1. Fees must be paid at the time of admission.
  2. Monthly fees should be paid at the beginning of each month.
  3. Fees once paid are non-refundable under any circumstances.
Attendance & Punctuality
  1. Students must attend classes regularly and arrive on time.
  2. Prolonged absence without intimation may result in cancellation of admission.
Discipline & Uniform
  1. Students must maintain discipline during all sessions.
  2. Wearing the official karate uniform is mandatory during classes and events.
Safety & Conduct
  1. Students must follow the instructor's guidance for their own safety.
  2. The academy is not responsible for any injury caused by negligence.
  3. Respect towards instructors, fellow students, and academy property is expected at all times.
Parental Involvement
  1. Parents are requested to enter their child's details directly into the software through the QR portal.
  2. A passport-size photo of the student must be uploaded.
Media Consent
  1. The academy may use photos or videos taken during training or events for promotional purposes.
  2. If parents do not wish to give consent, they must inform the academy in writing.
